HIPAA Privacy Policies

What is the difference between a hearing screening and a complete audiological evaluation?

A hearing screening is a short 10- to 15-minute test and is appropriate for individuals who are not sure whether they have a hearing problem. A complete audiological evaluation is a thorough test (45 minutes to 1 hour) that includes pure tone testing, speech testing, middle ear evaluation, and counseling. This is appropriate for individuals who already know they are having a hearing problem. Most insurance companies cover a complete audiological evaluation on a yearly basis.

Does the audiologist help explain the results of the evaluation and help me get hearing aids?

At the completion of the evaluation, the audiologist will explain the results and make recommendations.

Do I need to see a medical doctor/ENT doctor before my audiological evaluation?

If you feel you are having a medical problem (e.g. pain, discharge, etc.) in your ears then you should see a medical doctor first. If you have a history of earwax, then you should have it checked before your evaluation. We will give you a medical referral if we feel it is necessary after we complete the evaluation. If you need a hearing aid, you will need medical clearance.

Do you sell hearing aids? What manufacturers do you offer?

Yes, we sell hearing aids. Hearing aids can be expensive. Not everyone needs a high-end hearing aid. Our audiologists will discuss the options that are available to you. We offer a variety of manufacturers, so you can choose an instrument that best fits your lifestyle.

Are there any other services provided?

Yes, we offer rehabilitative services-aural rehabilitation, counseling.

Do you accept insurance?

Yes, we are providers of several insurance carriers. Call our office to check if we accept your insurance.

Is there parking available?

Yes.

Do I need to make an appointment, to be seen?

We do work by appointment only.

HIPAA Privacy Policies, Procedures, and Documentation Policy

Dr. Susan Antonellis is committed to protecting the rights of her patients. In compliance with the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”), the Health Information Technology for Economic and Clinical Health Act (“HITECH”) and applicable federal and state laws and regulations, this policy sets forth Dr. Susan Antonellis’ practice of implementing, enforcing, updating, and documenting its compliance with the HIPAA Privacy Policies and Procedures.

  • Dr. Susan Antonellis will implement policies and procedures that are reasonably designed to ensure compliance with the HIPAA standards, requirements, and implementation specifications.
  • Dr. Susan Antonellis will maintain documentation required for HIPAA compliance for a minimum period of six (6) years from the date of the creation of the document or the date the document was last in effect, whichever is later. Documentation will be retained in written or electronic form in accordance with Dr. Susan Antonellis' policies. 
  • Dr. Susan Antonellis will monitor changes to HIPAA and will promptly revise its policies and procedures and, if required, its Notice of Privacy Practices.
  • Dr. Susan Antonellis, as used in these policies, refers to all the professional, medical, and business entities and workforce members of those entities that interact with her practice. For example, insurance companies, hearing aid manufacturers. Some entities may also have additional operational procedures that are site specific, to properly implement these privacy practices.

Applicability

These HIPAA Privacy Policies and Procedures

 apply to all members of the Dr. Susan Antonellis 

community, including employees, 

residents, fellows, students, volunteers,

 trainees, vendors, contractors, 

consultants, and agents of Dr. Susan Antonellis.


 Policies that address patients’ rights apply for 

any patient of Dr. Susan Antonellis 

(as defined herein) at this Roslyn location.


Enforcement

Individuals who are found to be noncompliant with these HIPAA Privacy Policies and procedures may be subject to disciplinary action up to and including termination of employment or staff privileges. Having knowledge of inappropriate conduct and choosing not to report is, in itself, a violation of this Policy.


These policies shall remain in effect unless terminated or superseded by a revised and/or updated policy issued by this office.
